How do I enroll and how do I get help enrolling?

If you have been to the HealthSite and are ready to enroll, it's easy! You will need your employer ID (given when you or your employer filled out the online employer opt-in form), your SSN and home and business address. An email address is preferred, but not necessary. Simply click on one of the "enroll" buttons throughout the site and a form will pop up on your screen. Fill this out, pick your benefits, check the box that signifies you accept the terms and conditions (read them first) and click "submit". You will then be redirected to another site to make arrangements for monthly payment, Mastercard, Visa, AMEX or bank account.
